Cetrimide, or alkyltrimethylammonium bromide, is an antiseptic which is a mixture of three quaternary ammonium compounds: tetradonium bromide (TTAB or MITMAB), cetrimonium bromide (CTAB), and laurtrimonium bromide (DTAB or LTAB).[1] It was first discovered and developed by ICI and introduced under the brand name Cetavlon.[2] It is used as a 1-3 % solution for cleaning roadside accident wounds.[3] ICI also introduced Savlon, which was a combination of cetrimide and chlorhexidine.

Cetrimide is a quaternary ammonium antiseptic. Very dilute solutions of concentrations up to 1% are used for topical application. Adverse reactions after topical use have been reported earlier. In the index case a single application of pure cetrimide powder (100%) for pustules resulted in skin necrosis of the areas. This was due to the local irritant necrotic effect of cetrimide. No systemic side effects or toxicity was observed. This is because dermal absorption has been documented to be very low.

Indications and Dosage
Topical/Cutaneous
Dandruff, Scalp psoriasis, Seborrhoeic dermatitis
Adult: As 10% liq, in combination w/ undecenoic acid: Apply 1/2 to 1 teaspoonful 3 times during 1st wk, then twice wkly thereafter to the wetted scalp or hair. For application to other areas, apply sufficient amount as required and allow to remain in contact for 2 min.
Child: Same as adult dose.

Topical/Cutaneous
Superficial burns, Wound cleansing
Adult: As 0.5% cream: Apply directly to affected area. As 0.5% spray: Spray onto affected area from a distance of 2-4 inches bid or tid.
Child: Same as adult dose.

Incompatibility
Incompatible w/ anionic surfactants (e.g. soaps), bentonite, iodine, phenylmercuric nitrate, alkali hydroxides.

Contraindications
Hypersensitivity.
Adverse Reactions
Skin irritation and sensitisation. Rarely, burns (w/ concentrated soln).
Patient Counseling Information
Avoid application to the eyes, body cavities, and badly inflamed skin.
Drug Interactions
Enhanced bactericidal activity w/ preparations containing alcohol.

Action
Description: Cetrimide is a quaternary ammonium compound similar to cationic surfactants w/ bactericidal activity against gm+ve and some gm-ve bacteria. It dissociates in aqueous soln into a smaller inactive anion and a relatively large and complex cation that is responsible for the surface activity.

Also known as Cetrimide, Cetrimoni Bromidum and Tetradecyl trimethyl ammonium bromide (TTAB),  Strong Cetrimide Solution 40% BP Pharma with isopropyl alcohol (CAS No. 1119-97-7) consists of around 20% dodecyl trimethyl ammonium bromide and approcimately 80% trimethyl tetradecyl ammonium bromide, as well as approximately 7.5% volume by volume (v/v) isopropyl alcohol.

Cetrimide is a quaternary ammonium compound. Cetrimide was first introduced as a combined cleanser and skin antiseptic by Barnes (1942). Cetrimide combines excellent detergent properties and minimal toxicity with a useful antiseptic action. Cetrimide affects membrane permeability allowing ‘leaking’ of essential cell constituents leading to cell death. This medication is a skin antiseptic and disinfectant prescribed for seborrhoeic dermatitis and wound cleansing. The cream has a bactericidal activity against gram-positive bacteria and incompatible with soaps and other anionic surfactants.

These solutions are intended for use as an external irrigating solution and as an effective disinfectant against Gram-positive and Gram negative bacteria. Chlorhexidine is more effective against Gram-positive bacteria than Gram- negative bacteria, including some Pseudomonas and Proteus species that may be less sensitive.

Given the method for use of these solutions, absorption through the skin is minimal, no significant levels being found in blood.

Given its topical application, the potential toxicity of these products is very low.
Cetrimide Agar is a selective medium for the isolation and enumeration of Pseudomonas aeruginosa strains from biological samples of animal origin, pharmaceutical and cosmetic products. Its typical composition meets the combination of Pharmacopoeine prepared for the control of cosmetic products and NF EN ISO 22717 standards.

Storage
If you have this product at home, it should be kept in the original container, in a safe place, out of reach of children. A locked cupboard at least one and a half metres above the ground is a good place to store medicines.
Chlorhexidine and Cetrimide Irrigation Solution should be stored below 25 degrees C and protected from light.
Do not leave Chlorhexidine and Cetrimide Irrigation Solution in the car on hot days.
