As an antacid, E528 (Magnesium Hydroxide) is dosed at approximately 0.5–1.5 g in adults and works by simple neutralization, by which the hydroxide ions from the Mg(OH)2 combine with acidic H+ ions produced in the form of hydrochloric acid by parietal cells in the stomach, to produce water.
As a laxative, E528 (Magnesium Hydroxide) is dosed at 2–5 g, and works in a number of ways. 
First, Mg2+ is poorly absorbed from the intestinal tract, so it draws water from the surrounding tissue by osmosis. 
Not only does this increase in water content soften the feces, it also increases the volume of feces in the intestine (intraluminal volume) which naturally stimulates intestinal motility. 
Furthermore, Mg2+ ions cause the release of cholecystokinin (CCK), which results in intraluminal accumulation of water, electrolytes, and increased intestinal motility. 
Some sources claim that the hydroxide ions themselves do not play a significant role in the laxative effects of milk of magnesia, as basic solutions (i.e., solutions of hydroxide ions) are not strongly laxative, and non-basic Mg2+ solutions, like MgSO4, are equally strong laxatives, mole for mole.
Only a small amount of the magnesium from E528 (Magnesium Hydroxide) is usually absorbed by the intestine (unless one is deficient in magnesium). 
However, magnesium is mainly excreted by the kidneys so long-term, daily consumption of milk of magnesia by someone suffering from kidney failure could lead in theory to hypermagnesemia. 
Unabsorbed drug is excreted in feces; absorbed drug is excreted rapidly in urine.

In these products, it had been mostly used of the aluminum hydroxide in these products. 
However, it has now been converted to E528 (Magnesium Hydroxide) , mainly due to that in the thermoplastic resin, it can be increased of decomposition temperature to 350 ℃ or more with E528 (Magnesium Hydroxide); it is medically used for the control of gastric acid and laxatives; food additives, as a mineral additive, color protection agent, desiccant, alkali agent and aid of sugar production; used as a chemical reagent for measuring ammonium salt: Mg (OH) 2 + 2NH4Cl → MgCl2 + 2NH4OH.

EXTRACTION AND REFINING:

A strong chemical reagent, magnesium forms stable compounds and reacts with oxygen and chlorine in both the liquid and gaseous states. 
This means that extraction of the metal from raw materials is an energy-intensive process requiring well-tuned technologies. 
Commercial production follows two completely different methods: electrolysis of magnesium chloride or thermal reduction of magnesium oxide through the Pidgeon process. 
Electrolysis once accounted for approximately 75 percent of world magnesium production. 
In the early 21st century, however, when China emerged as the world’s leading magnesium producer, the low cost of labour and energy there allowed the Pidgeon process to be economically viable despite being less efficient than electrolysis.


PHARMACODYNAMICS:


As an antacid, E528 (Magnesium Hydroxide) suspension neutralizes gastric acid by reacting with hydrochloric acid in the stomach to form magnesium chloride and water. 
Magnesium Hydroxid is practically insoluble in water and does not have any effect until it reacts with the hydrochloric acid in the stomach. 
There, it decreases the direct acid irritant effect and increases the pH in the stomach leading to inactivation of pepsin. 
E528 (Magnesium Hydroxide) enhances the integrity of the mucosal barrier of the stomach as well as improving the tone of both the gastric and esophageal sphincters.
As a laxative, the E528 (Magnesium Hydroxide) works by increasing the osmotic effect in the intestinal tract and drawing water in. 
This creates distension of the colon which results in an increase in peristaltic movement and bowel evacuation.

MECHANISM OF ACTION:

The suspension of E528 (Magnesium Hydroxide) is ingested and enters the stomach. 
According to the amount ingested, the E528 (Magnesium Hydroxide) will either act as an antacid or a laxative.
Through the ingestion of 0.5-1.5 grams (in adults) the E528 (Magnesium Hydroxide) will act by simple acid neutralization in the stomach. 
The hydroxide ions from the E528 (Magnesium Hydroxide) suspension will combine with the acidic H+ ions of the hydrochloric acid made by the stomachs parietal cells. 
This neutralization reaction will result in the formation of magnesium chloride and water.
Through the ingestion of 2-5 grams (in adults) the E528 (Magnesium Hydroxide) acts as a laxative in the colon. 
The majority of the suspension is not absorbed in the intestinal tract and will create an osmotic effect to draw water into the gut from surrounding tissues. 
With this increase of water in the intestines, the feces will soften and the intraluminal volume of the feces will increase. 
These effects still stimulate intestinal motility and induce the urge to defecate. 
E528 (Magnesium Hydroxide) will also release cholecystokinin (CKK) in the intestines which will accumulate water and electrolytes in the lumen and furthermore increase intestinal motility.


SIDE EFFECTS:

Diarrhea may occur. 
If this effect persists or worsens, tell your doctor or pharmacist promptly.
If your doctor has directed you to use this medication, remember that he or she has judged that the benefit to you is greater than the risk of side effects. 
Many people using this medication do not have serious side effects.
Tell your doctor right away if any of these unlikely but serious side effects occur:

-symptoms of high magnesium levels (e.g., muscle weakness, slow/irregular heartbeat, slow/shallow breathing, mental/mood changes such as confusion)
-symptoms of dehydration (e.g., decreased urination, dizziness, extreme thirst, very dry mouth)
-stomach/abdominal pain
-bloody stools
-rectal bleeding

A very serious allergic reaction to this drug is rare. 
However, seek immediate medical attention if you notice any symptoms of a serious allergic reaction, including:

-rash
-itching/swelling (especially of the face/tongue/throat)
-severe dizziness
-trouble breathing

TOXICITY:

Common side effects include drowsiness or flushing (warmth, redness or tingly feeling).
Daily use of E528 (Magnesium Hydroxide) can result in fluid and electrolyte disturbances.
Excessive use of the laxative effects of E528 (Magnesium Hydroxide) may result in abdominal cramping, nausea and/or diarrhea.
In overdose, symptoms of gastrointestinal irritation and/or watery diarrhea may occur.
E528 (Magnesium Hydroxide) poisoning can result in hypermagnesemia which includes symptoms of: nausea, vomiting, flushing, thirst, hypotension, drowsiness, confusion, loss of tendon reflexes, muscle weakness, respiratory depression, cardiac arrhythmias, coma and cardiac arrest.
Not to be used in individuals with any form of kidney disease or renal failure, a magnesium restricted diet or with any sudden changes in bowel movement lasting over two weeks. 
Also not to be used in those individuals with abdominal pain, nausea, vomiting, symptoms of appendicitis or myocardial damage, heart block, fecal impaction, rectal fissures, intestinal obstruction or perforation or renal disease. 
Not to be used in women who are about to deliver as magnesium crosses the placenta and is excreted in small amounts in breast milk.
Using E528 (Magnesium Hydroxide) with aluminum hydroxide can decrease the absorption rate of these drugs.
Use of ibuprofen with E528 (Magnesium Hydroxide) can increase the absorption of the ibuprofen.
Use of E528 (Magnesium Hydroxide) with penicallamine, bisphosphates, ketoconazole, quinolones or tetracycline can decrease the absorption of these drugs.
Enteric-coated tablets can be prematurely released when taken with E528 (Magnesium Hydroxide).
Magnesium Hydroxid is important to routinely monitor levels of serum magnesium and potassium in patients using E528 (Magnesium Hydroxide). 
Serum magnesium levels are necessary to determine how much magnesium is being absorbed and how much is being excreted by the kidneys. 
Excessive diarrhea can occur from use of E528 (Magnesium Hydroxide) and thus it is important to also monitor serum potassium levels to ensure hypokalemia does not occur.


STORAGE:

Store at room temperature away from light and moisture. 
If you have any questions about storage, ask your pharmacist. 
Do not store in the bathroom. 
Keep all medicines away from children and pets.
Do not flush medications down the toilet or pour them into a drain unless instructed to do so. 
Properly discard this product when it is expired or no longer needed.
